Mojgan Sadeghi MD, is a board-certified family medicine physician and has been practicing for over 20 years. She has been practicing and living in Simi Valley for over 17 years. She very involved in her community and is an active admitting physician at Adventist Simi Valley Hospital. She is the department chair of family medicine, part of the medical executive committee, and part of the CIN at Adventist Simi Valley Hospital.
Sadeghi treats chronic medical conditions as well as emphasizes on prevention. Her main philosophy is to treat patient from head to toe. For that reason, she has developed a practice that is all-inclusive. SimiCare Medical Group offers a wide array of ancillary treatments that include woman and men health clinics, allergy testing and treatment, we have a psychologist for evaluation and treatment of mental issues, as well as nutritionist for support and augmentation of medical treatments of chronic conditions. SimiCare Medical Group also offers weight loss program, neuropathic supplement supports and as well as a wide range of skin treatments. She believes in complete care by increasing patient’s self-confidence as well as improving their mental health.

Shannon Chavez PA-C, MHS received a Bachelor's Degree from the University of California San Diego in 1998. In 2001, she left to the East Coast to pursue her Master's Degree in Physician Assistant Studies at Lock Haven University, in Pennsylvania.
Upon graduating in 2003, she started her career as a Physician Assistant in Internal Medicine in Lake Arrowhead, CA. She then spent 6 years working in the AV at two offices: one specializing in Pediatrics & Internal Medicine and the other a Family Medicine practice. Shannon has lived and worked in the Santa Clarita Valley for the last 15 years. She’s spent most of this time in a Primary Care and Urgent Care setting, but most recently worked in weight management.
Shannon is excited to return to Family Medicine. She loves having the opportunity to help patients manage both their chronic and acute medical conditions and getting to know her patients on a holistic level.
She joined SimiCare Medical in August, 2024, part-time. She loves being a part of this dynamic team!
On her days off, Shannon spends time volunteering at her children’s schools and supporting her three children at their events/competitions. She also loves cooking, traveling, fishing & camping.
Dr. Tousi has worked in the mental health and wellness field for over 15 years. She is passionate about supporting her patients towards finding authentic happiness and solidarity. Dr. Tousi works with adults, couples and families and has a range of specialities including depression, anxiety, stress management, relationship and intimacy, loss and grief, life transitions, parenting and personal growth.
When working with someone who is seeking support through therapy, Dr. Tousi aims to create a safe and comfortable environment. She works with her patients to understand what works best for them, depending on their needs and strengths.
Dr. Tousi believes that healing occurs when the patient is ready to move beyond their comfort zone, to work through their fears and challenges. That’s when they will gain insight and clarity to make lasting changes in their lives.
Karen Firestone is a Certified Functional Nutrition Counselor and Behavior Specialist who is passionate about helping people optimize their nutrition and lifestyle habits to improve their overall health and wellness. Karen’s philosophy is that health can only be built on a solid foundation. As such, she aims to identify the underlying causes of a patient’s health concerns and target those root causes using a holistic, nutrition and lifestyle-based approach.
Karen has spent nearly 20 years using therapeutic diet and lifestyle strategies to reverse her own autoimmune symptoms. More recently, dietary interventions have helped her address peri-menopausal symptoms, sleep challenges, resistant weight loss, and blood sugar imbalance. The profound difference this made in her life led to her desire to help others achieve the same success. Part of her philosophy includes the strong belief that everyone deserves to feel empowered and supported all through their healing journey. Knowing firsthand how challenging this journey can be, Karen approaches every patient with patience, compassion, and understanding.
Through nutrition counseling, Karen’s goal is to provide patients with a powerful understanding of their body and its highly individualized needs. Based on this understanding, she helps them identify which habits are serving their health goals, which are not, and why. Her behavioral background gives her a unique perspective in the field of functional nutrition. Not only can she help people develop successful habit- change strategies based on scientific behavioral principles, but she also understands the need for ongoing support to achieve long-lasting change.
